Due to the current conditions of COVID-19, we are asking that all members please return your ballot and, if needed, proxy via the email southernmaineyouthhockey@gmail.com no later than 6:30 pm on April 27th, 2021 for your vote to be counted in this year’s election of officers.

Ballot and proxy must be received through the email (scanned copy of ballot and proxy or electronically filled out with digital signature on this form) southernmaineyouthhockey@gmail.com by 6:30 pm on April 27th, 2021.

If you need to have another member parent submit your ballot on your behalf, fill out the proxy on the second sheet. 

Members eligible for voting in this election must have a registered player in either: the 2020-21 Season or the upcoming 2021-22 season.
